The Therapeutic Play Foundation (TPF) helps families in Los Angeles County who often don’t get the care they need. They focus on whole-person care, which means supporting people’s physical, mental, and emotional health. TPF works in Service Planning Areas 3, 5, 6, and 8, helping people with complex needs—like members experiencing homelessness, with kids in foster care, people with serious mental illness or disabilities, pregnant and new parents, and families facing unfair health outcomes.

TPF started working with PATH (Providing Access and Transforming Health) by receiving help through the Technical Assistance (TA) Marketplace. This support helped them grow stronger as an organization. They learned how to improve their systems, train staff, and take part in Medi-Cal’s transformation. Now, TPF is a TA Marketplace vendor, which means they help other community-based organizations (CBO) learn how to use the Marketplace to grow and keep going. They focus on workforce training, health equity, and designing care that supports the whole person.
TPF also received funding through PATH’s CITED program (Capacity and Infrastructure Transition, Expansion, and Development).
This helped them open a permanent service center in a high-need area of L.A. County. They also built a strong digital system, including:
- A custom Electronic Health Record (EHR) system
- A live phone bank for outreach
- Workflows that follow health rules and prepare them for QHIO (Quality Health Information Organization) participation
Now, TPF uses LANES QHIO to share health data in real time and coordinate care. With help from the Data Exchange Framework (DXF) grant, they are fully ready to connect with other health systems and deliver high-quality care.
TPF also works closely with the community. They have formal partnerships that let them host monthly resource fairs, where people can get food, diapers, and bus passes. These events help more people sign up for Medi-Cal and get support in their own neighborhoods. TPF is also growing its Community Health Worker hubs by partnering with the Department of Mental Health to bring services to schools. These programs include wellness activities like art and gardening.
TPF’s story shows how PATH can help organizations grow, build strong systems, and support whole-person care. Thanks to PATH, TPF and the groups they support are making a lasting difference for Medi-Cal providers and the communities they serve.
