Strengthening Behavioral Health Care Services Through Providing Access and Transforming Health (PATH)

The following success story describes experiences across multiple PATH initiatives.

Turning Point Community Programs (TPCP) is committed to empowering people of all ages and identities through quality behavioral health and social services.

As a PATH Capacity and Infrastructure Transition, Expansion, and Development (CITED) awardee and PATH Technical Assistance (TA) Marketplace recipient, TPCP has bolstered its capacity to provide high-quality Enhanced Care Management (ECM) and Community Supports to the members it serves. PATH has enabled TPCP to enhance service coordination, build stronger community partnerships, and provide comprehensive, person-centered care that makes a lasting impact. 

One TPCP member, experiencing homelessness and significant mobility challenges, was struggling to meet basic needs. With the support of their ECM Lead Care Manager, they were able to regain stability and improve their quality of life. 

The Lead Care Manager provided both a standard and electric wheelchair, helping the member regain mobility and independence. They also connected the member to housing resources and assisted with their application for Social Security Disability Income, laying the foundation for long-term stability. Additionally, the Lead Care Manager coordinated food assistance, transportation, and access to warming centers.  

Support for this member went beyond their immediate needs. When their pet required life-saving surgery, local veterinary providers and community donors collaborated to ensure the necessary care was provided. 

With a safe, stable place to live and ongoing support, the member has reduced their reliance on the wheelchair, improving their mobility and quality of life. 

Through PATH, TPCP has strengthened its ability to meet complex needs with a holistic, coordinated approach. “The additional training, resources, and partnerships fostered through PATH have allowed us to improve care delivery, overcome barriers, and achieve better outcomes for the people we serve,” said Alexis Bernard, Chief Development Officer of TPCP.
