Sanctuary Centers of Santa Barbara (SCSB) and Premier Valley Medical Group (PVMG) have partnered to offer mental health care to individuals and their families through their Integrated Health Clinic (IHC). As a Providing Access and Transforming Health (PATH) Capacity and Infrastructure Transition, Expansion, and Development (CITED) awardee, SCSB turned its IHC vision into a reality.

Through PATH CITED funding, the IHC makes health care more accessible, providing hundreds of members with psychiatric, therapy, and primary care services each year. The clinic uses the Collaborative Care Model (CoCM) to combine behavioral health and primary care into a single, coordinated plan. This approach ensures clients receive comprehensive care for both their physical and mental health needs.
The IHC improves access to timely mental health services and reduces the need for emergency care in hospitals, offering a consistent pathway to stability and recovery for people facing complex health challenges. Ongoing support and coordinated care have led to fewer hospitalizations, better management of chronic conditions, and improved health outcomes for vulnerable Santa Barbara community members.
