Saint John’s Program for Real Change: Building Capacity Through PATH Technical Assistance

Saint John’s Program for Real Change (SJP) is enhancing its ability to serve the Sacramento community through the Providing Access and Transforming Health (PATH) Technical Assistance (TA) Marketplace. With support from 43 Strategic Consulting (43SC), SJP has focused on preparing to deliver California Advancing and Innovating Medi-Cal (CalAIM) services by identifying relevant Community Supports, evaluating financial feasibility, and strengthening internal infrastructure.

43SC’s targeted, hands-on support has been a game changer. Their collaborative approach has helped SJP clarify goals, organize workstreams, and make informed strategic decisions. The engagement lead fosters inclusive dialogue, the project manager ensures progress stays on track, and the financial and technical expert provides guidance for long-term sustainability.

This partnership came at a pivotal moment in SJP’s growth. 43SC helped define the project scope, align internal teams, and secure the resources needed to meet critical milestones. As SJP COO Kara Fiscus noted, “The roadmap felt overwhelming at first, but 43SC made it manageable. They helped us turn a big goal into something real and achievable.”

Director of Government Funding Dora Noegel added, “They broke down the work into manageable pieces while keeping the big picture in view. They took time to understand our organization and worked alongside us, respecting our culture and becoming part of the team.”

Thanks to this collaboration, SJP has successfully contracted with one Medi-Cal managed care plan and is finalizing agreements with two more. These efforts are already delivering real-world results:

  • A client received emergency housing within 72 hours after experiencing domestic violence.
  • Another client achieved sobriety, earned a high school diploma, secured full-time employment, and received support to obtain permanent housing.

Reflecting on the partnership, Fiscus shared, “I’ve tried to find the right words—‘smartest consultants,’ ‘best working relationship,’ ‘impeccable partners’—but nothing fully captures it. I’m proud of what we’ve accomplished together.”

SJP’s continued collaboration with 43SC reflects the trust and confidence built across the organization. Through the PATH TA Marketplace, SJP has transformed a bold vision into a practical, sustainable program that strengthens its ability to serve the community.
