Mom’s Meals is a Community Supports Medically Tailored Meal provider whose mission is “improving life through better nutrition at home.” Mom’s Meals strives to improve the health outcomes of people covered by Medi-Cal by delivering medically tailored meals, nutrition counseling, and produce and pantry boxes directly to people’s doorsteps.
As an active member in the Providing Access and Transforming Health (PATH) Collaborative Planning and Implementation (CPI) initiative, Mom’s Meals regularly connects with a network of Enhanced Care Management (ECM) providers, community-based organizations, and other Community Supports agencies across California to identify individuals who may benefit from nutrition support to maintain and improve their health and well-being.
As a medically tailored meal provider serving Californians for more than 20 years, Mom’s Meals is proud to be one of the hundreds of organizations working collaboratively to identify and resolve ECM/Community Supports needs and gaps and ensure individuals are receiving the services they need as quickly and efficiently as possible under Cal-AIM.
