Restorative Partners and NHHA are Transforming Justice-Involved Care With Help From PATH

Restorative Partners is a nonprofit organization founded in 2011 in San Luis Obispo, with a dedicated team of 32 staff and more than 100 volunteers. The organization supports justice-involved individuals and their families through in-custody programs for both youth and adults, reentry services, and five transitional recovery reentry homes. Restorative Partners also offers outpatient drug and alcohol treatment, anger management, and Enhanced Care Management (ECM) and Community Supports. Additionally, it operates a small business, The Bridge Café, which further supports its mission of community reintegration and healing.

With help from National Healthcare and Housing Advisors (NHHA) through the California Department of Health Care Services’ (DHCS) Providing Access and Transforming Health (PATH) Technical Assistance (TA) Marketplace, Restorative Partners launched ECM and Housing Navigation Community Supports. NHHA saw strong opportunities for the organization to contract for these services, helping them build a steady funding stream and connect with the Medi-Cal health plan, CenCal. 

Since launching ECM in July 2024, Restorative Partners has reached 386 individuals, enrolling 59 people in ECM, and providing housing-related Community Supports to more than 60 members. To date, the organization has billed more than $318,000 in reimbursable California Advancing and Innovating Medi-Cal (CalAIM) services and secured an additional $200,000 in Medi-Cal Incentive Payment Program  funding. These outcomes illustrate how PATH support has strengthened Restorative Partners’ organizational capacity and enhanced their ability to deliver integrated, equitable care to the communities they serve.
