Mysti’s Adult and Family Services helps people in Monterey, Contra Costa, Los Angeles, and Santa Cruz counties find stable housing, health care, social services, and jobs. They offer Enhanced Care Management (ECM) to all populations and provide Community Supports like Nursing Home Diversion, housing services, Short Term Post-Hospitalization Housing, Recuperative Care, and Respite Services.

Mysti’s is engaged in the Providing Access and Transforming Health (PATH) Collaborative Planning and Implementation (CPI) initiative and works closely with other Medi-Cal providers in the Coastal CPI collaborative. By sharing ideas and building partnerships, they’ve been able to send and receive more referrals to help clients get the services they need.
One client came to Mysti’s after being released from prison in 2023. They had struggled with substance use and felt let down by other programs. Mysti’s focused on building their confidence and helping them find a full-time job and permanent housing. After getting hired, they worked with their case manager to apply for a Section 8 housing voucher. When they needed surgery, Mysti’s helped schedule their care and made sure they had a safe place to recover while they extended their voucher.
After healing, they found a home and shared:
“I’m proud to say that I now have permanent housing. I have a beautiful one-bedroom, one-bathroom apartment with a great kitchen and a patio where I can cook carne asada and start rebuilding my confidence.”
Thanks to PATH, Mysti’s has been able to grow their ECM and Community Supports programs, helping more people reach their goals and improve their health through stable housing.
