PATH is Helping Organizations Form Strategic Partnerships with Managed Care Plans

The following success story describes experiences across multiple Providing Access and Transforming Health (PATH) initiatives.

Kings View has been providing behavioral health and social services to individuals with limited resources since 1951. Through engaging in the PATH initiative, Kings View is investing in their IT infrastructure to provide more coordinated services to members and building strategic partnerships with Medi-Cal managed care plans (MCPs) across California.

PATH Capacity and Infrastructure Transition, Expansion, and Development (CITED) funding has enabled Kings View to develop Enhanced Care Management (ECM) and Community Supports for several populations of focus across Fresno, Kings, Madera, Merced, Shasta, and Tulare counties. Since receiving their CITED award, Kings View has scaled these benefits and provided:

  • 6,146 ECM outreach services
  • 3,910 ECM enrolled services
  • 6,296 Housing Transition Navigation Services
  • 1,014 Housing Tenancy and Sustaining Services
  • 4,720 Recuperative Care (Medical Respite) services
  • 11 Sobering Center Services.

Kings View also provided more than $460,000 in Housing Deposits.

“Each of these achievements represents an unparalleled opportunity for Kings View to help Californians in need, and none of this would be possible without PATH,” said Kings View representative Eric Rosen.

To better navigate the California Advancing and Innovating Medi-Cal (CalAIM) environment and learn best practices for delivering enhanced care, Kings View also joined the PATH Collaborative Planning and Implementation (CPI) Initiative and built successful strategic partnerships with Anthem Blue Cross, Central California Alliance for Health, Health Net/Cal Viva, Kaiser Permanente, and Partnership HealthPlan of California.

Kings View also received free support from Health Management Associates (HMA), through PATH’s Technical Assistance (TA) Marketplace, to successfully map the financial feasibility of providing ECM and Community Supports long after the initial investments. This assistance has been critical to ensuring services are available to Medi-Cal members now and well into the future.

Finally, Kings View teamed up with Qualifacts to co-develop and implement an electronic health record system to track, record, and submit ECM and Community Supports data to each Medi-Cal managed care plan. PATH has opened the door for Kings View and Qualifacts to form a long-term partnership that will benefit organizations across California by helping them implement electronic health record systems for enhanced care services.
