Person-Centered Healthcare for High-Needs Residents

Homeward Bound of Marin is using PATH Capacity and Infrastructure Transition, Expansion and Development (CITED) funding to better serve the people in their housing program who need the most help. The nonprofit is pleased to share how PATH CITED funds are strengthening their capacity to provide quality, coordinated Enhanced Care Management (ECM) and Community Supports services to the families and individuals they serve.

ECM is a person-centered approach that helps Medi-Cal members with complex health needs by focusing on everything that impacts their well-being, not just medical problems:

  • A woman in her 50s who came to our Jonathan’s Place shelter in San Rafael from a local encampment was struggling with substance use disorder and had a history of hypertension and strokes. During the intake process, our case manager noticed the client appeared very weak and asked if she needed medical help. The client initially was hesitant but agreed to have her blood pressure taken. Our team helped get her to an emergency room due to her blood pressure being dangerously high. We maintained contact with her during her 3-day hospital stay, during which she was diagnosed with atrial fibrillation. After the client was stabilized and returned to our shelter, our case manager worked with her to pursue a housing pathway and meet other needs, such as getting documents ready for housing applications, scheduling and keeping medical appointments, and connecting with mental health and substance use disorder treatment providers.  In September, the client signed a lease to her new housing unit upstairs from the shelter at our Jonathan’s Place permanent supportive housing program, which serves the most vulnerable chronically homeless adults. The client is thrilled to have a safe and permanent place to call home and thankful for the progress she has made with the support of our staff. She is committed to sustaining that progress in her new home.
  • A resident at our La Casa Buena permanent supportive housing program in Corte Madera has been better managing his chronic lung disease thanks to the housing support and care we provide. This includes arranging for him to see a pulmonologist several times and working with him to adjust his medications with input from his doctor. We are grateful our program allowed him to breathe easier and feel better
