Through Providing Access and Transforming Health (PATH) Capacity and Infrastructure Transition, Expansion, and Development (CITED) funding, a day health center has significantly strengthened its capacity to deliver quality, equitable, and culturally responsive Enhanced Care Management (ECM) to its members.
PATH CITED Round 2 funding has enabled an adult day health center to hire and train staff who have similar lived experiences as the members they serve and can speak to members in their language. This investment in culturally competent staff and overcoming language barriers has improved the organization’s ability to respond to member crises and address their complex medical, housing, and other needs.
ECM providers can more effectively help members navigate doctors’ appointments, as well as answer questions about health insurance or benefits. The adult day health center reports that one member who was facing homelessness was able to access secure housing within a week thanks to the rapid response of their ECM team. Another member was connected to a specialist by their ECM team to diagnose a concerning growth on their body. The ECM team was able to attend the member’s doctor appointment, translate communications from the health care provider, and assist the member with follow-up questions.
These examples are just a snippet of what the adult day health center’s ECM team has accomplished since the initiation of their PATH partnership. The organization is thankful for CITED funding and the connections it has been able to make through PATH, as this support has empowered the center to make a tangible difference in their community.
