Food Health Collective (FHC) is transforming health care by integrating food and nutrition through its Food Health Rx program. FHC aims to improve members’ lives through lasting behavior changes that reduce hospitalizations and help manage chronic conditions through food and culturally relevant nutrition education.

As an active member in the Providing Access and Transforming Health (PATH) Collaborative Planning and Implementation (CPI) initiative, FHC regularly connects with a network of Enhanced Care Management (ECM) and Community Supports providers and other community-based organizations to share and contribute expertise in culturally responsive nutrition solutions.
Through PATH, FHC participates in the South Bay and Indian Health collaboratives. By actively engaging with peers, FHC is gaining valuable insights into the successes and challenges experienced in the communities they collectively serve, allowing them to adapt and refine their approach.
