PATH is Helping Providers Serve Children and Youth With Complex Needs

The following success story describes experiences across multiple Providing Access and Transforming Health (PATH) initiatives.

As a provider serving the Children and Youth Population of Focus (POF), CRF Healthy Connect Youth Services (HCYS) San Diego connects members with access to medical specialists, durable medical equipment, and medications. HCYS also supports individuals with complex medical and behavioral health conditions and meets social service needs, like housing, food, utilities, benefits and entitlements, and transportation.  

PATH Capacity and Infrastructure Transition, Expansion, and Development (CITED) funding has enabled HCYS to increase Enhanced Care Management (ECM) staffing and capacity to reach more members throughout San Diego County.  

HCYS is also involved in the PATH Collaborative Planning and Implementation (CPI) initiative, where best practices and resources are discussed and shared between providers during collaborative convenings.  

“Both funding and collaborative efforts have played a significant role in the successes of our ECM members, as described in the following client success story,” said Michael Hammel, Vice President of Care Management for HCYS. 

“One of our members has been enrolled in HCYS since September 2023. At the time of enrollment, the individual asked for help with finding employment opportunities and low-cost gym memberships. Our Lead Care Manager (LCM) connected the member to free employment counseling/career development opportunities and two free gym memberships. The member got a full-time job in July 2024 and has been employed since. The member also began attending college part-time and uses the free student gym memberships to work out regularly. 

“The member says thanks to the resources and support provided by HCYS, they are more independent, positive, and healthier. The member’s success highlights the value of ECM by showing how the benefit can address multiple needs at once. It also shows how ECM can support members at any stage of their treatment or recovery.” 

HCYS is grateful for the PATH initiative that is sustaining their work with children and youth.
