Avenidas Social Work Services is empowering older adults across California to navigate the health care system and receive the services and support they need to live dignified lives. As an Enhanced Care Management (ECM) provider, they assist family caregivers with navigating complex legal, financial, and health care systems and connect individuals to community resources like food banks, transportation, and social supports.

Through the Providing Access and Transforming Health (PATH) Technical Assistance (TA) Marketplace, Avenidas is accessing free tools and resources to help them better coordinate personalized, holistic care for California’s older population.
