As a Providing Access and Transforming Health (PATH) Capacity and Infrastructure Transition, Expansion, and Development (CITED) awardee, Adventist Health is using funding to bolster its capacity to support participants receiving Housing Deposits, Housing Navigation, and Recuperative Care support.

Since launching its California Advancing and Innovating Medi-Cal (CalAIM) programs in its Community Health Department, Adventist Health has housed 134 individuals, with 80 percent remaining housed after placement. Before participating in CalAIM, a single case manager handled housing navigation, medical care, and continuous case management.
“We did not have the staffing to follow up with people once they were housed, which led to more instability,” said Ronni Duncan, Care Management Manager at Adventist Health. “PATH CITED funds have allowed us to expand staffing, fill service gaps, and let case managers focus on their specific roles.”
“Most of all, these programs have helped build community and provide hope and healing to people who may not have had access to the healing support we offer,” Ronni added.
The Benefits of ECM for Patients with Complex Needs
Adventist Health shares another story from one of their clients named “Frank.” The individual was seen in an Adventist Health emergency department 25 times over five months while experiencing homelessness. His symptoms were wide-ranging and numerous diagnostic and treatment services were required.
Frank’s transformation began in October 2024, when an Adventist Health lead care manager (LCM) enrolled him in Enhanced Care Management (ECM) benefits. Four days later, he was in a temporary shelter and eventually was placed on a high-priority list for housing. His LCM also obtained a motorized scooter and shower chair for him and connected him to a primary care physician. On his birthday, she brought him a sandwich and balloons.
In five months of ECM and housing services, Frank had only two emergency visits.
“This program does work. We have seen amazing results in terms of the impact and care delivery provided to our patients,” says Yesenia Mock, system administrative director of accountable care organizations and value-based care. “We love to see the outcomes of what CalAIM has brought us.”
