VIDEO: Redwood Community Services Is Advancing Whole-Person Care Through PATH CITED Support

For more than 50 years, Redwood Community Services (RCS) has been a trusted partner for individuals and families across Northern California, providing compassionate, community-based services that support health, well-being, and independence. Through a broad range of behavioral health, social services, youth and family support programs, and housing-related services, RCS works to improve outcomes for some of the region’s most vulnerable populations.

In 2023, RCS launched Enhanced Care Management (ECM) and Community Supports under California Advancing and Innovating Medi-Cal (CalAIM), creating a new pathway to care for some of Northern California’s most vulnerable residents. Before CalAIM, there was no sustainable funding source for housing navigation services, leaving many individuals without the support needed to find and secure stable housing. The addition of CalAIM, coupled with funding received through the Providing Access and Transforming Health (PATH) Capacity and Infrastructure Transition, Expansion, and Development (CITED) initiative, transformed what was possible.

CITED funding supported ECM and Community Supports staffing, helped establish office space for new service positions, and provided vehicles that expanded in-person outreach to people living in isolated and rural communities across Humboldt, Lake, and Mendocino counties.

The impact has been significant. Clients who previously struggled to access health care, behavioral health services, housing assistance, and other critical resources are experiencing improved quality of life and greater stability. ECM care coordinators serve as a central hub, connecting clients to providers, care teams, and community resources, all while working closely with housing navigators to ensure comprehensive, coordinated support.

Since receiving the PATH CITED award, RCS has supported hundreds of community members throughout rural Northern California. The investment has expanded services and created lasting opportunities for individuals and families to achieve greater health, stability, and hope for the future.
