Participation in the Providing Access and Transforming Health (PATH) Technical Assistance (TA) Marketplace has significantly supported the growth of the Family Services Alliance (FSA)—a coalition of more than 40 Family Resource Centers (FRCs) and other family-serving community organizations across San Francisco.

With help from their TA vendor, FSA has set clear goals for how to be an active participant in Medi-Cal’s transformation, and is now moving from learning about the initiative to actively working on contracts with Medi-Cal health plans and developing referral pathways throughout the city.
They’ve gained a deep understanding of how Enhanced Care Management (ECM) and Community Supports can make a real difference for the children and families they serve. From March to August 2025, FSA’s TA vendor provided:
- Trainings for both potential California Advancing and Innovating Medi-Cal (CalAIM) providers and CalAIM referral partners
- Strategy sessions on integrating ECM and Community Supports into family support work
- “Readiness Roadmapping” for members
- Office hours with members
- Empathy Mapping
- And more.
Through working with their TA vendor, FSA completed the Community Goal Setting and Needs Assessment project. So far, this project has resulted in:
- More than 15 FSA organizations agreeing to become ECM and Community Supports providers or referral partners.
- Providers beginning contracting for services which is expected to help more than 3,000 children and youth in San Francisco County access ECM.
- Helping five organizations apply for the fourth round of CITED funding.
- More than 25 FSA staff who have learned about ECM, Community Supports, and other health care services through training efforts.
This project also helped FSA create a special “sub-Hub” model. This model will help build the Medi-Cal ecosystem for children, youth, and families in San Francisco by supporting FSA organizations with peer learning and capacity building to ensure Medi-Cal services are delivered from trusted and well-integrated CBOs in the community. The model will also increase the number of bottom-up referrals for CalAIM services coming from county partners and other existing family support infrastructure to ensure that all eligible families are connected with these services. This is all in support of the FSA’s goals to ensure that all families thrive.
