PATH Funding Supports Outreach to a Broad Range of Enhanced Care Management and Community Supports Providers in Yolo County

The Camden Coalition helps people, organizations, and communities provide care to individuals with complex needs through equitable, quality systems for health and social services. As a Providing Access and Transforming Health (PATH) Collaborative Planning and Implementation (CPI) Facilitator, Camden Coalition builds relationships between health plans, Enhanced Care Management (ECM) and Community Supports providers, and community-based organizations (CBOs) to assist with California Advancing and Innovating Medi-Cal (CalAIM) transformation.

Through the PATH CPI initiative, organizations form networks, learn more about the services they can provide to Medi-Cal members, and identify ECM and Community Supports gaps and opportunities. DHCS-designated facilitators in all regions of the state are responsible for coordinating these discussions during monthly virtual and in-person collaborative convenings.

The Camden Coalition is a facilitator for the Merced, Southeast, and Coastal collaboratives. The Coalition highlighted several efforts from the Yolo County CalAIM Connections Event and Resource Fair, which it helped organize:

Leaders at the Yolo County Health and Human Services Agency (HHSA) are regular participants in the Southeast collaborative and identified an opportunity to strengthen county-wide alliance on Medi-Cal transformation. At the request of the Yolo County HHSA, the Camden Coalition partnered with sponsors First 5 Yolo and the Yolo County Office of Education to hold an in-person CalAIM Connections Event and Resource Fair.

The event, attended by community-based ECM and Community Supports providers, health plans, and Yolo County HHSA Director Nolan Sullivan, provided foundational information about ECM, Community Supports, and PATH resources.

Tony Kildare, Branch Director of the Child, Youth, and Family Branch at Yolo County HHSA,  provided welcoming remarks prior to Camden Coalition’s “Introduction to Medi-Cal Transformation” session. Representatives from managed care plans (MCP) Kaiser Permanente and Partnership HealthPlan of California provided an overview of CalAIM contracting and referrals and answered questions.

About 90 representatives from county agencies, county contractors, CBOs, and MCPs participated in the Resource Fair. Nearly two dozen tables representing ECM and Community Supports providers, MCPs and MCP Network Lead Entities, the CPI Facilitator team, and the Technical Assistance (TA) Marketplace provided information and resources to attendees.

The event’s success encouraged Yolo County to expand and scale its current ECM pilot with support from the TA Marketplace and PATH Capacity and Infrastructure, Transition, Expansion, and Development (CITED) funding.

Events attendees said they found networking and building relationships to be the most beneficial parts of the event. Many left the event with a list of next steps, new contacts for community referrals, and ideas for strengthening their organizations through PATH initiatives. The county plans to co-host a future CalAIM Resource Fair to engage community members directly. The commitment of local leaders and providers to improve health outcomes for people with complex health needs was clear, making it a valuable and inspiring day.
