Korean Community Services (KCS) uplifts underserved communities in Orange County by providing community programming, health care, and social services. Providing Access and Transforming Health (PATH) Capacity and Infrastructure, Transition, Expansion, and Development (CITED) funding has helped KCS expand its capacity to deliver comprehensive care and support vulnerable community members.
Through CITED funding, KCS invested in targeted staff training; as a result, their lead care managers (LCM) are better positioned to guide patients through the California Advancing and Innovating Medi-Cal (CalAIM) housing program. Specialized training sessions equipped KCS with the skills necessary to navigate the complexities of housing procurement, manage claims efficiently, and provide tailored support to patients who are experiencing homelessness or at risk of becoming homeless.
The training program emphasized crucial areas, such as understanding CalAIM guidelines, communicating effectively with property management, and managing housing deposits and rent payments. KCS’s LCMs were also trained in motivational interviewing and case management, ensuring they could provide compassionate and complete support to patients transitioning into stable housing.
This training led to substantial outcomes for KCS. For example, the trained staff helped a member get a job and move into stable housing. In another case, staff helped a member find an apartment after experiencing homelessness for three years. KCS covered all necessary fees and deposits.
These triumphs are direct results of the LCMs’ enhanced capabilities. Thanks to training funded by PATH CITED, LCMs are better prepared than ever to handle the unique challenges their patient populations face. This professional development has improved KCS’s operational efficiency and significantly impacted the lives of the organization’s patients, helping them achieve the stability and independence they deserve.
