Serving Orange County’s Unhoused Population with Help from PATH Funding

Housing for Health Orange County (HHOC) is a nonprofit organization working to support a network of unhoused and housing providers. HHOC exists to provide centralized administrative and financial support to these providers and to assist in co-creating best practices and standards of care to ensure quality outcomes for their clients.

As a Providing Access and Transforming Health (PATH) Capacity and Infrastructure Transition, Expansion, and Development (CITED) awardee, HHOC and its partner agencies have accelerated skilled hiring and secured an office space, significantly enhancing their capacity to serve more community members.

Specifically, HHOC hired three positions, including Chief Administrative Officer, Operations Director, and Administrative Assistant, and filled Referral Coordinator and Billing Specialist positions. Their partner organizations retained a total of seven key positions and 16 new staff members, including lead care managers, supervisors, and case managers.

HHOC shares the following client success story, highlighting how organizational investments made through PATH funding allows them to continue to serve members in need:

“After six years of homelessness, our client was connected with Friendship Shelter through CalAIM. The client received a housing voucher via Coordinated Entry. Through Friendship Shelter’s various programs and collaboration with outreach navigation and Enhanced Care Management (ECM), the client’s needs were met, and the client found a suitable living unit. Despite systemic barriers, the client’s determination led to secure housing in the client’s hometown, where strong community ties exist. Now housed, the client continues to receive support from HHOC’s permanent housing team and ECM to ensure ongoing success.” – HHOC staff member.
