Heritage Group Homes helps people in Los Angeles, San Bernardino, and Riverside counties by offering Enhanced Care Management (ECM) services to adults and youth. They focus on people experiencing homelessness, individuals who frequently visit the hospital, and people living with serious mental health or substance use disorders.

As a Providing Access and Transforming Health (PATH) Capacity and Infrastructure Transition, Expansion, and Development (CITED) awardee, Heritage Group Homes is working to better connect adults with mental illness to the care they need. They used the PATH CITED funding to build a system that:
- Follows a clear process for making referrals that meet state standards.
- Tracks how quickly and successfully people get connected to services.
- Has staff who work closely with care teams to guide members through the system.
Thanks to this work, Heritage Group Homes says that 91 percent of members referred to outside providers got help within 14 days. Their records passed all reviews with no issues, and they have seen fewer repeated services and better coordination across care providers.
