Recovery Road Transitions to a Fully Integrated ECM and Community Supports Provider Through PATH

Recovery Road is an Orange County community-based organization that helps people find stability and healing. Their goal is to support individuals on their journey to recovery, wellness, and housing. Each month, Recovery Road serves more than 6,500 Medi-Cal members with services, such as housing navigation, food access, mental health resources, recovery programs, and help with legal forms. Many of the people they work with are experiencing homelessness, leaving jail or prison, or facing serious health and social challenges.

As Recovery Road has grown, they’ve become a strong provider of Enhanced Care Management (ECM) and Community Supports. These services help people who need more than medical care, such as support with housing, mental health, or navigating the health care delivery system. With help from the Providing Access and Transforming Health (PATH) Technical Assistance (TA) Marketplace, Recovery Road has improved and expanded the way they deliver care. They’ve received training, tools, and guidance from the California Department of Health Care Services (DHCS) and their TA Marketplace Vendor, Health Management Associates (HMA), which helped them strengthen their programs and serve more people.

“Everyone at Recovery Road is so grateful for the vital support we’ve received from DHCS during this transformative journey,” said Angelo Barsotti, Executive Administrator at Recovery Road. “DHCS’ guidance and investment through the PATH TA Marketplace have been instrumental in helping us move from grassroots operations to a fully integrated ECM and Community Supports provider.”

Recovery Road’s food pantry now serves more than 1,500 people a week. When someone comes in for a food box, they may also leave with mental health support, a referral to a recovery program, help finding housing, or a warm handoff to other services. This whole-person approach makes it easier for people to get the full range of help they need, all in one place.

“Thanks to DHCS and HMA, we’ve done more than meet regulatory milestones. We’ve learned how to fully integrate Medi-Cal-aligned care coordination into everything we already do,” said Barsotti.

Behind the scenes, Recovery Road has built a stronger foundation. They’ve improved how they track care, protect personal information, and work with health plans. They are hiring more care coordinators and launching a secure system to better track results and share information with partners. These changes mean that Recovery Road can support members more effectively while meeting Medi-Cal’s quality, equity, and safety standards.

Recovery road is also pleased to work with Full Circle Health to serve Kaiser Permanente Medi-Cal members. Full Circle offers tools and support that make it easier for organizations like Recovery Road to connect members with care.

Barsotti says Recovery Road believes there should be a trusted community organization like theirs in every California county. “Looking ahead, we see this partnership as the beginning of a much larger impact. With Full Circle by our side, we are positioned to reach even more high-frequency emergency department users, people facing homelessness, justice-involved members, and others with complex, high-touch needs. Together, we can deliver better outcomes faster and with greater dignity.”

Recovery Road is actively credentialing providers, completing their PAVE enrollment, onboarding new care coordination staff, and launching an electronic health record system that will allow them to securely track member outcomes and share data with their partners. What began with public commitment is now backed by formal infrastructure, and that evolution wouldn’t have been possible without PATH.

“Helping others isn’t just something we do. It’s who we are,” Barsotti beamed. “Now, thanks to PATH, the TA Marketplace, and visionary partners like Full Circle Health, we are delivering that mission with more strength, reach, and the capacity to serve people who need it most.”
